The following information will be printed in a class yearbook for distribution to your classmates at Law Alumni Weekend 2009 and will be posted online on a password-protected site accessible only to members of your class.  If you prefer to not have your submission published in both formats, please make the appropriate selection at the bottom of this form. 

Please submit your class yearbook form no later than Friday, April 2, 2010, to ensure its inclusion in the printed version of your class yearbook.
